If your last name begins with A-L you are assigned Case Study #1 If your last name begins with M-Z you are assigned Case Study #2 Case Study #1: Fritz Matthews, age 72, has had uncontrolled hypertension for the past 2 years. His healthcare provider has tried different antihypertensives to control his disease. Mr. Matthews has been admitted to the hospital four times in the last 6 months. This is his fourth admission with acute heart failure. The physician explained to Mr. Matthews that hypertension that is not controlled can cause an excessive workload to the heart, thus decreasing the heart’s ability to pump efficiently. Mr. Matthews has recently lost his wife, and is living alone. He states that he forgets to take his medication at the prescribed times. He also eats a lot of frozen dinners and does not have the energy to cook his meals. Mr. Matthews has gained 10 pounds since his last admission. He has had difficulty sleeping at night and uses two pillows due to shortness of breath. He also reports that he frequently gets up at night to urinate. Mr. Matthews complains of fatigue during the day and is unable to even walk to his mailbox. He also tells the nurse that he is tired even in the AM upon rising. Case Study #2: Gladys Hammer is an 83 year old female who is seen in the emergency department with complaints of shortness of breath and fever. Her vital signs are T. 102 F, P115, and irregular, shallow respirations at 35, B/P 168/66. Her height is 5ft 3 in and weight is 96 pounds. On assessment, her skin is very dry and warm. Her color is dusky. Scattered wheezes and rhonchi are heard throughout all lung fields. Her chest is hyperresonant to percussion. A pulse oximeter is applied and the O 2saturation is reading is 88%. Mrs. Hammer has a past history of emphysema for 10 years, complaints of shortness of breath on exertion, and has a chronic cough productive of thick, grayish sputum. Her spouse passed away 5 years ago and she has lived with her two adult sons since then. _______________________________________________________________________________________________________________________ Discussion Requirements This discussion forum requires that the student create an initial post before being able to see other student posts. Once you make your first post, your classmates’ posts will appear and you can respond as directed below. Students must first post a topic before they are allowed to view or reply to other student’s posted topics. Your initial response should be posted no later than midnight on Day 3. Then post 2 responses to peers on two additional days during the week. The minimum participation is 3 posts (initial + 2 responses) on 3 different days. Your input should be in your own words, demonstrating your understanding, and comprehension of the topic. Bottom Line: Be sure to at least meet the minimum posting requirements...you cannot just log in, post one day and be done. You are not required to reply to everyone, but to read all posts during the weekly discussion. I am primarily looking for quality posts that further the discussion.
